PROVO, UTAH (September 5, 2012) - - To introduce residents to the new facility and highlight services provided, the Utah County Health Department will hold an Open House at their new American Fork office on Thursday, September 13th from 10 a.m. to noon. Community officials will join UCHD staff in a ribbon cutting and opening ceremony, as well as facility tours and refreshments.

"The new facility has been very well received," said Lance Madigan, UCHD public information officer. "Some people are stilling just learning about it, and that they don't have to come into Provo for many of our services. We hope the Open House will provide a chance to more people to see what is offered there."

New UCHD office in American Fork

The office is now open Monday through Friday from 8 a.m. to noon and 1:00 to 4:30 p.m. (closed regular holidays). For questions, individuals can call 801-851-7333. "We are excited to be serving our north county residents closer to their homes and businesses, and hope the office will be useful and convenient for those citizens," said Dr. Joseph Miner, UCHD director.

UCHD walk-in services available to everyone in the general public include:

  • Birth and death certificates
  • Immunizations (child, adult, and travel)
  • Daycare facility application/inspection requests
  • Food establishment permits (annual and temporary)
  • Food handler renewal/replacement
  • Mass gathering permits
  • Radon / meth test kits
  • Tuberculosis testing
  • Water Sample pick-up

Appointments will be necessary for some additional services, including:

  • Child car seat and booster seat safety checks (call 801-851-7035)
  • "Stepping-On" adult fall prevention classes (801-851-7035)
  • Youth tobacco cessation classes (801-851-7035)
  • WIC Nutrition Services (801-851-7320)
