Temporary Food/ Farmers Market Permit Come in person to the Environmental Health Office to submit your application. You should set aside approximately 30 minutes for this process. Your application should be submitted at at least 14 days before the event. We are located in the Health & Justice Building at 151 South University Ave, Suite 2600, Provo, Utah 84601. Our office hours are Monday through Friday from 7:30am to 5:30 pm. NOW OPEN!! Limited Services, some by appointment* only. Open 8 am to 4:30 pm (closed for lunch 12-1pm). Application Instructions
