Children with Special Health Care Needs

Children with Special Health Care Needs (CSHCN) is part of the Utah Department of Health, Division of Family Health Services. Specialized diagnostic and support services for children with physical or developmental disabilities and/or special health care needs are provided and assist families in obtaining treatment for these children.

CSHCN can assist families in dealing with Medicaid and Supplemental Security Income (SSI) programs. Families can be referred to local resources and other assistance as needed.

Information and referral is available to all families at no charge. Children are eligible for evaluation if they are under 18 years of age, are residents of Utah, have (or may have) physical or developmental disabilities, special medical needs, or learning/behavioral problems.

CSHCN offers diagnostic team evaluation, referral to appropriate community resources, and care coordination/case management for children with complex medical needs. Help is also available in obtaining specialized treatment for children with the following conditions: Cleft lip and/or palate, cerebral palsy, orthopedic defects, congenital heart defects, myelodysplasia and seizure disorder.

Evaluation services available through Children with Special Health Care Needs include: pediatric nursing, audiology, developmental/behavioral pediatrics, psychology, occupational and physical therapists, speech/language evaluation, social work and orthopedics. Cardiology, neurology, genetics and oral/facial clinics are offered through CSHCN's contracts with Primary Children's Medical Center.

Parents, teachers, physicians, public health nurses, friends or relatives can make a referral to CSHCN. It is important that you discuss your application to CSHCN with your primary care doctor.

Families are billed according to their income. Whenever possible Medicaid and private health insurance will be billed.

To make a referral, or to find out more about CSHCN, contact 801-584-8510. That is a Salt Lake Office (they closed the Utah County office due to budget issues in January of 2009.
